"An important aspect of the role of PPG members is to identify ways of improving the patient experience within Chatsworth Road Medical Centre Practice. When members discuss the outcomes from patient surveys we sometimes identify an area of need and undertake specific project work which will help improve services."

Mental Health Support & Awareness - (May 2018)

The PPG are currently working on a Mental Health Support & Awareness project – if you are interested in getting involved in this project please leave your contact details with the receptionist and they will pass these onto the PPG group who will contact you directly.

Cardiac Support Groups in the Chesterfield Area

Lots of us end up with heart related health issues as we get older and as well as support from the medical centre it might also be beneficial to join a support group who can provide such things as regular exercise routines and social activities. The Patient Participation Group has produced the attached guide to the three local Cardiac Support Groups affiliated to the BHF. Before you join any such group please check with one of the doctors about whether this is the right thing to do and perhaps arrange to visit each of the groups to see which one is best for you.

You may have heard of the project called "21st century - Joined Up Care". This is a major project being managed by our regional comissioning group that will bring together all the services that look after patients with complex needs.
