Social Prescribing Link Worker

Social prescribing takes a non-medical approach to improving health and wellbeing, by exploring local activities and services in the community. It is designed to support people with a range of social, emotional and practical needs, and gives people to time to focus on “what matters to them”.

Social prescribing gives you time to think about what is important to you and identify ways to improve your health and happiness.

Together we will develop an action plan and assist you to access services and activities within your community.

Your GP or Nurse can refer you.

Physio

First Contact Physiotherapy Service

Do you have a musculoskeletal problem?

You are likely to be directed to our First Contact Physiotherapist (FCP) based at the surgery. 

They are physiotherapists with enhanced skills.  They can help patients with musculoskeletal issues by providing expert assessment for conditions affecting bones, muscles, and joints, including longer-term issues such as low back pain, neck pain, and arthritis.

FCPs will assess and diagnose your condition, provide expert advice on how best to manage it, and refer you on for further treatment where appropriate. This may include physiotherapy, specialist services, or investigations such as x-rays, MRI scans, and blood tests.

You can book directly with an FCP, without needing to see a GP first, by contacting the practice reception team.
